The Opportunity

1x Full Time Fixed-Term Contract Opportunity for 6 Months located in Geraldton
Suitable applicants will be placed in a pool from which appointments will be made for this or similar future vacancies (fixed term or permanent) that arise within the next 12 months across the Department of Communities.
As a Regional Intensive Support Coordinator within Disability Division, you will be responsible consultating, coordinating, providing support and advice regarding individuals with exceptionally complex support needs, their family and carers who are experiencing (or are at risk of experiencing) multiple and interrelated conditions or factors which contribute to an intensity of support need.
In this position you will be responsible for (but not limited to):
- Providing specialised advice, support, and coordination for staff and stakeholders managing individuals with exceptionally complex disability-related needs, including high-risk cases.
- Strengthening internal capability by offering guidance, education, and resources to support effective local responses and informed assessments.
- Maintaining accurate, up-to-date records, protocols, and documentation using appropriate systems and technology.
- Collaborating with external partners, identifying service gaps, driving improvements, sharing best practice, and representing the department in inter-agency forums.
- Contributing to planning, processes, and evaluations, while identifying trends and recommending policy or practice enhancements.
Positions located in regional areas may be eligible for an additional ‘District Allowance’ on top of regular base salary.
