ABOUT THE ROLE
Fremantle Hospital Mental Health Service is seeking to appoint suitably experienced and enthusiastic individuals to the position of Clinical Nurse. As part of a multidisciplinary team, the Clinical Nurse provides clinical and professional expertise in the delivery of recovery focused mental health intervention within the Home Treatment Team (HTT). In this role you will provide assessment, time limited management and support within the areas of speciality and advanced practice to consumers with complex and mental health needs, their families and carers, within and external to the service. You will also facilitate and promote patient safety and quality of care.
Area Profile: Fremantle Hospital Mental Health Service (FHMHS) provides recovery focused mental health services. It works in partnership with consumers and their carers and acknowledges that each individual is an expert in their own recovery. FHMHS provides services to the residents of Fremantle, Cockburn and Melville. It offers adult services to people aged 18 to 65 years and older adult services for people over 65 years. Our services include: acute inpatient mental health services and community mental health services (including outpatient clinics and continuing care programs).
