About the Opportunity:

We are currently seeking a skilled and dynamic individual to appoint to a permanent, full-time Team Leader position with the Clinical Treatment Team, which forms part of the City East Community Mental Health Service. In this position, the successful applicant will be responsible for the provision of leadership, direction, and management of the human, financial and material resources within the Treatment Team in accordance with the policies, philosophies, objectives and goals established through consultation with staff and in accordance with the directions of the East Metropolitan Health Service and other relevant health services. Key aspects of the role include, but are not limited to;

  • Accountable for the standard of care and for the coordination of nursing and multidisciplinary services provided to clients. 
  • Ensures collaboration and engagement with key stakeholders to ensure patient-centred care and evidence-based service delivery for this population.
  • Provides advice and consultancy to the Program Manager and Service 3 Co-Directors.
  • Coordinates and directs daily operations of the team in consultation with the relevant Program Manager.
  • Performs duties in accordance with the EMHS Vision and Values, WA Health Code of Conduct, Occupational Safety and Health legislation, the Disability Services Act and the Equal Opportunity Act and Government, WA Health, EMHS and Departmental / Program specific policies and procedures.

About the City East Community Mental Health Service:

The City East Community Mental Health Service provides access to adult outpatient mental health services including treatment, urgent assessment and intervention.

The City East Community Mental Health Service offers the following services:

Assessment and Treatment Team

The Assessment and Treatment team (ATT) is the first point of contact for all referrals to adult community mental health services within the City East catchment area. The multi-disciplinary team includes psychiatrists, nurses and allied health staff who help to provide triage, assessment and short-term care coordination and treatment options for consumers up to a maximum of 10 weeks.

Clinical Treatment Team

The Clinical Treatment Team (CTT) receives all referrals via the ATT and offers multi-disciplinary mental health assessment and care coordination for consumers who require specialist care and treatment options for more than 10 weeks.

City East Older Adult Mental Health Service

The City Older Adult Mental Health Service provides a comprehensive recovery-focused mental health service to Older Adult consumers within the inner city and eastern metropolitan region of Perth. The service provides an assessment and/or care coordination service to consumers aged 65 or over (45 for Aboriginal and Torres Strait Islander consumers) who have mental health issues, or who are experiencing severe behavioural and psychological problems associated with dementia. The team consists of psychiatrists, community nurses, social workers, occupational therapists and psychologists.

Mobile Clinical Outreach Team

The Mobile Clinical Outreach Team (MCOT) provides services through clinical assessment, community triage and treatment for rough sleepers. The team is street present and works closely with specialist homeless services, mainstream mental health services, drug and alcohol services and primary care services to deliver a coordinated model of care.
