About the Opportunity

Are you a passionate leader with a commitment to supporting older people and vulnerable members of the community to live independently? Exmouth Health Service is seeking an experienced and community-focused Support Services and Aged/Community Care Coordinator to lead the delivery of high-quality home and community care services across Exmouth.

In this diverse and rewarding role, you will coordinate the Care services that enable clients to remain safely and comfortably in the residential aged care their own homes and connected to and their community. You will be responsible for overseeing service delivery, ensuring compliance and quality standards are met, and managing a multidisciplinary team providing community-based support services and residential aged care services.

The role also manages the provision and quality of Support Services at Exmouth Health Service, including catering, cleaning, laundry and orderly services. As a key leader, you will provide operational oversight, staff management, and continuous service improvement to ensure exceptional outcomes for customers, residents and healthcare consumers.

Key Responsibilities

  • Coordinate and oversee the delivery of Aged / Community Care, ensuring clients receive high-quality, person-centred support that enables them to remain living independently in their homes and community.
  • Conduct client assessments, develop and review care plans, and work closely with clients, families, and service providers to ensure individual care needs are met.
  • Lead and manage Support Services operations, including catering, cleaning, laundry, and orderly services, ensuring compliance with quality, safety, and infection control standards.
  • Manage service budgets, reporting, funding requirements, data collection, and administrative processes to support efficient and sustainable service delivery.
  • Provide leadership and support to staff through recruitment, rostering, performance development, training, and workforce planning.
  • Ensure compliance with relevant legislation, accreditation standards, policies, and continuous improvement initiatives across Community Aged Care and Support Services.
  • Build and maintain effective relationships with clients, families, community stakeholders, advisory groups, and health service leadership to support positive outcomes for the Exmouth community.

Form and define what will be assessed to determine your suitability for the role:

  1. Relevant qualifications and experience , Extensive experience in Health, Community Services or a related discipline, together with demonstrated experience in human services and community care environments.
  2. Strong leadership, communication and people management skills, with experience leading multidisciplinary teams, managing performance, workforce planning, recruitment and rostering.
  3. Excellent organisational, operational and financial management capability, including time management, budget oversight, reporting, resource management and the ability to deliver efficient and effective services.
  4. A client-centred approach and sound knowledge of relevant legislation and standards, including community aged care principles, Equal Opportunity, Disability Services, Work Health and Safety requirements, and possession of a current C Class Driver's Licence.
