This is a Fixed Term Full Time appointment until 30/9/2027 with the possibility of extension(s).

Eligibility: This temporary position is to be filled from within WA Health by secondment, temporary deployment (acting) or temporary contract. Prior to applying, applicants must seek conditional approval for release from their current Line Manager. 

The Project Manager will lead the implementation, governance and support evaluation of the Nursing Plus Program, a key initiative within the Department of Education-led Connected Community Schools (CCS) Trial. Working across health, education and community sectors, the role provides strategic and operational leadership to a small team of Health Navigators to deliver program outcomes and support organisational and government priorities.

The CCS Trial aims to improve access to services, supports and activities for local communities through better utilisation of school sites. Participating schools have identified a need to strengthen support for vulnerable children and young people, particularly in accessing health assessments, treatment services and coordinated care.

The Nursing Plus Program is being piloted across four CCS schools, including three metropolitan sites and one WA Country Health Service site. The program uses a Health Navigator model to provide intensive, coordinated and culturally responsive support for vulnerable and at-risk children, young people and their families.

This role will be responsible for building and maintaining strong partnerships across health, education and community services, supporting implementation of an innovative service model, and ensuring robust governance, performance monitoring and reporting. The program seeks to streamline access to health services and contribute to improved health, wellbeing and educational outcomes for children and young people.
