About the Opportunity:

The Clinical Nurse Specialist (CNS) is responsible for contributing and overseeing delivery of quality patient care within the Home Hospital service. The CNS works collaboratively to improve and promote desired outcomes for patients through the development of standards of practice, implementation of changes to clinical practice, initiating research and quality improvement activities; and the provision of a consultation service within their area of expertise/assigned area to all customers within the Hospital and Health Service. Key duties of this role include, but are not limited to;

• The role acts as an expert resource for staff and patients in the delivery of Home Hospital care.  Liaising with Home Hospital service stakeholders inclusive of Ward Teams, Multidisciplinary Teams, and Community based services and clinicians.
• Responsible for facilitating and liaising with inpatient areas to lead and manage the provision of patient flow for the health service.
• Role models a positive organisational culture in line with EMHS vision and values.
• Provides a leadership role, coordinating and implementing quality improvement activities, recognising National Safety and Quality Health Service Standards.

About East CareConnect and the Home Hospital Service:

Our new virtual hospital is delivering hospital-quality treatment in people’s homes through traditional medical care and technology. 

A new arm of our health service dedicated to harnessing technology to provide hospital-quality care in the community was officially launched in June. Transformative, state-of-the-art and patient-centred, East CareConnect marks a major step forward in how we can deliver health care and brings our virtual services together under one banner.

For the first time, patients have this year been able to choose hospital treatment at home under the East CareConnect Home Hospital service for some conditions which in the past would have required them to physically be at a hospital site. 

Virtual services are an important part of ongoing healthcare evolution. The Home Hospital Service provides opportunities to free up physical hospital beds for those who need them the most. East Care Connect is a dynamic, growing service that is laying the foundation for a more sustainable, accessible, and equitable healthcare future.

Our Home Hospital service provides hospital-quality care in patients’ homes for various acute conditions that would previously have required an in-hospital stay. The service offers comprehensive, multidisciplinary care through in-person visits by health professionals and virtual means, with remote monitoring of vital signs through wearable technology.

The Home Hospital service can manage up to 70 patients across three sites, Royal Perth, Midland and Armadale hospitals. As part of its operations, Home Hospital is building innovative patient pathways for acute medical, post-surgical, respiratory and complex conditions that allows care delivery in the patient's home
