About the Opportunity:
We are seeking skilled and dynamic individuals to appoint to Clinical Nurse positions with the Armadale Community Mental Health Service. We currently have two permanent positions available. One is Full-Time, and the other is Part-Time.
About the Role: As part of a multidisciplinary team, the Clinical Nurse provides clinical and professional expertise to ensure comprehensive evidence-based nursing care is delivered to patients in a community environment. This role facilitates and promotes patient safety and quality of care, provides guidance to junior nursing staff, and provides assessment, time limited management & support within the areas of speciality & advanced practice to consumers with complex and mental health needs, their families and carers, within & external to the service. Key aspects of this role include, but are not limited to;
- Provides comprehensive mental health assessments throughout all stages of consumer care.
- Sets priorities for the management of consumer care in conjunction with other health professionals as appropriate.
- Provides comprehensive medium to long term care coordination to consumers, including assessment, intervention and evaluation.
- Supports the delivery of safe patient care and the consumers’ experience including participation in continuous quality improvement activities in accordance with the requirements of the National Safety and Quality Health Service Standards and other recognised health standards.
