The Older Adult Health Hubs are a new model of care, providing safe and quality care for older adults through integrated, community-based intervention. This specialist-led older adult hub delivers and integrates with services that focus on proactive and responsive care, risk prediction and early intervention.
The SMHS Older Adult Health Hub:
- Optimises older adult independence through maintaining and improving function, growing health literacy, supporting choice autonomy and scaffolding and supporting carers
- Provides coordinated, comprehensive multidisciplinary care
- Supports liaison between primary care and SMHS services.
About this role:
We are seeking a highly organised and proactive Administrative Assistant to join our Cockburn Older Adult Health team. This role is ideal for someone who enjoys supporting business operations, managing multiple priorities, and delivering exceptional administrative support. The successful applicant shall provide clerical and administrative support to the South Metropolitan Older Adult Health Hub to support the effective day to day operation of the service.
