Develop and apply your clinical expertise within our specialist paediatric care environment.

We currently have a Fixed Term (1 FTE) appointment, for up to 12 months, available as Full Time or Part Time, with possibility of extension(s) and permanency.

Applicants assessed as suitable through this process will be considered for other Full Time, Part Time and Casual P2 Pharmacist vacancies within the team (within the next 12 months).

About the Role  

The Child and Adolescent Health Service is seeking experienced Senior Pharmacists to join our team provide high-quality, evidence-based clinical pharmacy services within a specialist paediatric hospital environment across Perth Children’s Hospital and Neonatology, and community-based locations in the Perth Metro area.

As a Senior Pharmacist, you work closely within a team of multi-disciplinary health professionals to optimise outcomes for children and young people and their families. You will monitor and review patients' medication profiles, identify and manage medication-related risks, provide expert drug information and apply your advanced clinical knowledge to support the safe, rational and cost-effective use of medicines.

You will also contribute to the ongoing development, evaluation and improvement of pharmacy services and evidence-based clinical pharmacy practice

Key responsibilities include:

  • Providing evidence-based advice and drug information to patients, carers and health professionals.
  • Participating in clinical reviews, case conferences and multidisciplinary team meetings.
  • Providing medication counselling and education to patients and families to support safe and effective use of medicines.
  • Supporting the safe supply, storage, handling and administration of medicines across clinical areas.
  • Supervising or undertaking the preparation of dispensed and manufactured medicines, including intravenous admixtures, cytotoxic medicines and parenteral nutrition, where required.
  • Supporting the professional development of the Pharmacy team through clinical orientation, supervision, education, mentoring and support of pharmacists, interns and students.
  • Initiating and contributing to quality improvement, research and service evaluation activities.
  • Ensuring pharmacy practice complies with relevant legislation, professional standards, hospital requirements and medication management policies.

Requirements:

A tertiary qualification in Pharmacy and eligibility for registration with the Pharmacy Board of Australia is required for this role.
