Position Profile and Role

This is a Fixed-Term, Sessional appointments for 7 months, with the possibility of extension(s) and/or permanency.

We are currently recruiting to a:

• Consultant Geriatrician at 0.4 FTE (8 sessions per fortnight)
• Consultant Geriatrician - Clinical Lead at 0.2 FTE (4 sessions per fortnight)

These positions are an opportunity to lead the multidisciplinary team to develop and provide a new, subacute geriatric medicine, model of care to patients across the South Metropolitan catchment. In collaboration with other Medical Consultants (Psychiatrist, Palliative Care Physician, General Practitioner) they promote safety and quality of care, and integration with other hospitals, community- based health services, non-government, and community organisations as well as general practitioners, developing pathways to care. 
The clinical lead position will work closely with the Director of Clinical Services, Director of Community Services, Service Manager, other Health Service Providers and Clinical Leads, and Department of Health to provide clinical and strategic leadership to the Older Adult Health Hub model of care.

Older Adult Health (OAH) Hubs are designed to support older adults to achieve optimal health through safe, high-quality, integrated, community-based care. The hubs are specialist-led and operate through a combination of virtual and physical services, delivering and coordinating proactive and responsive care. This includes illness and injury prevention, risk prediction, early intervention, and timely access to appropriate services.  

The SMHS Older Adult Health Hub:

• Optimises older adult independence through maintaining and improving function, growing health literacy, supporting choice autonomy and scaffolding and supporting carers
• Provides coordinated, comprehensive multidisciplinary care
• Supports liaison between primary care and SMHS services 

SMHS delivers quality, safe and effective hospital and health services within a catchment area stretching more than 3300 square kilometres across the southern half of Perth.
