We are seeking applications from enthusiastic and experienced Clinical Nurses or Nurse Managers to join Residential Care Line to provide high quality outreach visiting services to patients in residential aged care facilities, in a supportive and collaborative team environment.

What is our Clinical Nurse Manager - Residential Care role?

The Residential Care Line Service based in Sir Charles Gairdner Hospital (SCGH), is Western Australia’s only State-wide, Nurse Practitioner led outreach service providing support to residential aged care staff to provide care in place.

Our passionate team of nurses work collaboratively with residential aged care home (RACH) staff and practitioners, providing clinical excellence, expert knowledge and education to empower RACH staff to improve the quality-of-life of older people living in residential care, supporting their choice to receive care in their ‘own home’.

We are seeking enthusiastic and innovative Nurses from primary care or hospital settings with aged care expertise to join our team as leaders, responsible for providing:

  • Expert aged care clinical consultation, and advice to staff working in residential aged care, through on-site visits, phone or via telehealth.
  • Education and mentorship aimed at up-skilling and empowering staff:
    • working in the residential aged care setting in Western Australia and
    • Residential Care Line Service team members.
  • Advanced clinical leadership in line with organisational core values of Care, Respect, Innovation, Teamwork & Integrity; acts as a role model, providing leadership, support, team building and change management through respect, recognition, and collaboration.

Applicants will be required to support a roster that operates from 0800- 1800 Monday - Sunday.

Whilst this selection process will initially be used to fill the above vacancy, it may also be used to fill other "similar" vacancies throughout the health service or the wider public service. Both of these options remain valid for a period of twelve (12) months from when the authorised delegate endorses the recruitment decision.
