We are seeking to appoint a Coordinator Complex Care to the Complex Needs Coordination Team Mental Health Expansion (CoNeCT MHE).

We are offering the position through a Recruitment Pool on Fixed Term Full Time or Fixed Term Part Time basis for 12 months, with the possibility of extension(s) or permanency.

In this role you will provide an advanced level of hospital and community focused care coordination to an identified “at risk” client target group of people and their carers’, whose care and support needs are assessed as complex.

You will be responsible for being a metro-wide resource on care coordination especially in the areas of mental health and disability. This position will require you to network and link with key stakeholders across the care continuum of acute care, sub-acute care and community settings. This will include education and liaison to actively build relationships across services working with the client group. 

You will also actively lead the development and implementation of an evaluation process, including recording, collecting and reporting key performance information. In addition, you will conduct quality improvement activities in line with the CCC program development and future planning to ensure evidence-based practice healthcare.
