We are seeking applications from enthusiastic and experienced Nurse Practitioners to provide high quality outreach visiting services to patients in residential aged care facilities, in a supportive and collaborative team environment.
We have 2 immediate vacancies:
A) 1 x Permanent Part-Time at 0.9 FTE (72 hours per fortnight) Job Share arrangements may be considered and
B) 1 x Fixed Term Full-Time for 6 months, with the possibility of extension(s) and / or permanency.
What is our Nurse Practitioner role?
The Residential Care Line Service based in Sir Charles Gairdner Hospital (SCGH), is Western Australia’s only State-wide, Nurse Practitioner led outreach service providing support to residential aged care staff to provide care in place.
Our passionate team of nurses work collaboratively with residential aged care facility (RACF) staff and practitioners, providing clinical excellence, expert knowledge and education to empower RACF staff to improve the quality-of-life of older people living in residential care, supporting their choice to receive care in their ‘own home’.
We are seeking enthusiastic and innovative Nurse Practitioners, from primary care or hospital settings with aged care expertise to join our team as leaders, responsible for providing:
- Expert aged care clinical consultation, and advice to staff working in residential aged care, through on-site visits, phone or via telehealth.
- Education and mentorship aimed at up-skilling and empowering staff:
- working in the residential aged care setting in Western Australia and
- Residential Care Line Service team members.
- Advanced clinical leadership in line with organisational core values of Care, Respect, Innovation, Teamwork & Integrity; acts as a role model, providing leadership, support, team building and change management through respect, recognition, and collaboration.
Applicants will be required to support a roster that operates from 0800- 1800 Monday - Sunday.

Eligibility to Apply: Australian citizenship or permanent residency is an essential requirement for applicants to be considered for permanent positions in the public sector. To be eligible for a fixed term appointment you must have documentary evidence of your entitlement to live and work in Australia for the period of the contract.
Whilst this selection process will initially be used to fill the above vacancy, it may also be used to fill other "similar" vacancies throughout the health service. In addition to this, should the successful applicant decline or vacate the advertised vacancy, then the next most suitable applicant may also be selected from this process. Both of these options remain valid for a period of twelve (12) months from when the authorised delegate endorses the recruitment decision.
