Practitioners to provide high quality outreach visiting services to
patients in residential aged care facilities, in a supportive and
collaborative team environment.
This is a Recruitment
Pool for Permanent Full Time, Permanent Part Time, Fixed Term Full
Time and Fixed Term Part Time appointments. Fixed Term appointments
will be made with the possibility of extension(s) and/or
permanency.
Suitable applicants will be placed
in a pool from which appointments may be made when similar
vacancies occur. Appointments may be made from this pool until 30
June 2026.
What is our Nurse
Practitioner – Residential Care Line
role?
The Residential Care Line Service based in
Sir Charles Gairdner Hospital (SCGH), is Western Australia’s
only State-wide, Nurse Practitioner led outreach service providing
support to residential aged care staff to provide care in
place.
Our passionate team of nurses work
collaboratively with Residential Aged Care Facility (RACF) staff
and practitioners, providing clinical excellence, expert knowledge
and education to empower RACF staff to improve the quality-of-life
of older people living in residential care, supporting their choice
to receive care in their ‘own home’.
We are seeking enthusiastic and innovative
Nurse Practitioners, from primary care or hospital settings with
aged care expertise to join our team as leaders, responsible for
providing:
- Expert aged care clinical
consultation, and advice to staff working in residential aged
care, through on-site visits, phone or via
telehealth.
- Education and mentorship
aimed at up-skilling and empowering staff:
-
- working in the
residential aged care setting in Western Australia
and
- Residential Care
Line Service team members.
- Advanced clinical
leadership in line with organisational core values of Care,
Respect, Innovation, Teamwork & Integrity; acts as a role
model, providing leadership, support, team building and change
management through respect, recognition, and
collaboration.
Applicants will be required to support a
roster that operates from 0800- 1800 Monday - Sunday.
