This is an Open Recruitment Process for Permanent Full Time, Permanent Part Time, Fixed Term Full Time and Fixed Term Part Time appointments with the possibility of extension and/or permanency. Appointments may be made from this process until 31/10/2024.

Position Profile and Role: Connect Care Program for Kids provides care coordination and support for children with medical complexity across Western Australia. The program aims to support children and their families in their own community and acts as an interface between home and hospital with the aim of reducing hospital appointments and emergency presentations.
What we do -
- Create partnerships with a child’s healthcare team – including their GP, community and local care providers, hospital and healthcare providers and non-government organisations.
- Act as a central point of contact for the child.
- Help parents and caregivers coordinate access to the services their child needs.
- Help parents and caregivers coordinate their child’s appointments where possible.
- Support, guide and empower you parents and caregivers to manage your child’s healthcare needs.
- Make sure parents and caregivers understand their child’s healthcare needs, so they can make informed decisions about their childs’ health.
We have multiple opportunities available in the Connect Care team currently.
Applicants must possess the following skills and experience:
- Eligible for registration with the Nursing and Midwifery Board of Australia
- Strong communication, advocacy and interpersonal skills.
- Current Australian Drivers’ Licence
