About this role
Home Link is a nursing service based at Sir Charles Gairdner Hospital which provides acute care at home or in a dedicated clinic for patients from both Sir Charles Gairdner and Osborne Park Hospitals. The service can access all multidisciplinary community services to help support early discharge from hospital.
The Registered Nurse practises as a Registered Nurse in keeping with the Australian Nursing and Midwifery Board Registration Standards and Nursing Practice Decision Flowchart. In this role you will provide comprehensive evidence-based nursing care to patients as part of a multidisciplinary team. You will uphold and function within the core values of the organisation and nursing values of Care, Respect, Innovation, Teamwork & Integrity.
Successful applicants will be part of the Home Link nursing team as a rotating shift worker and will gain experience in all areas.
This role reports to the Nurse Manager.
