South
Metropolitan Health Service (SMSH) is seeking a Service
Manager to lead the team at our Older Adult Health
Hub (OAHH) in Cockburn.
Position
Description
The Older
Adult Health Hubs are a new model of care for the older adult
providing safe and quality care through integrated, community-based
intervention supporting individuals in achieving optimal health.
These specialist-led older adult hubs are part virtual, part
physical and will deliver and integrate with services that focus on
proactive and responsive care, illness and injury prevention, risk
prediction and early intervention.
The Service
Manager will provide strategic and operational leadership for the
Older Adult Health Hub. They will manage and support a
multidisciplinary team including Care Navigators, Nursing, and
Allied Health staff, and will facilitate integrated care by
developing relationships with a variety of stakeholders including
medical specialities, nursing, allied health and primary and
community care providers.
The Service
Manager role is instrumental in refining and developing the service
model to ensure it meets its core objective: to provide older
adults with chronic and complex conditions access to coordinated,
person-centred care in the community that supports wellness,
reduces avoidable hospitalisations, and improves health and social
outcomes. Key responsibilities include overseeing day-to-day
operations, which encompass the effective management of physical
infrastructure, human resources, and financial assets. The role
also involves supervising staff, fostering strategic partnerships
with community services, identifying and addressing service gaps,
and ensuring the Hub remains responsive to the evolving needs of
patients and primary care providers. In addition, the Service
Manager is accountable for meeting reporting requirements,
including performance metrics, resource utilisation, and service
outcomes, to support continuous improvement and transparency. This
position will be based at the Cockburn Integrated Health and
Community Facility located in Success and may require travel across
the SMHS catchment area.
SMHS delivers
quality, safe and effective hospital and health services within a
catchment area stretching more than 3300 square kilometres across
the southern half of Perth.

months from when the authorised delegate endorses the
recruitment decision.
