About
the Role
The Child and Adolescent
Health Service (CAHS) is seeking a Care Navigator
to join our team and ensure infants and families receive timely,
coordinated and family-centred care following referral from
Western Australian Newborn Hearing Screening (NBHS)
Program. This is a unique opportunity to make a meaningful
difference in the lives of children and families during a critical
stage of development.
This position is primarily a
coordination and support role, involving regular contact with
families via telephone, telehealth and occasional face-to-face
appointments.
Working closely with
families, audiologists, ENT specialists, Hearing Australia, early
intervention providers and community services, you will provide
ongoing guidance, support, education, and advocacy throughout the
diagnostic and intervention pathway.
Your focus will be on
helping families understand hearing screening and diagnostic
outcomes, navigate services, attend appointments, and access the
supports they need to achieve the best possible outcomes for their
child.
You will play a key role in
ensuring families feel supported, informed, and confident
throughout their journey.
Key responsibilities
include:
- Maintaining regular
contact with families to promote engagement and continuity of
care throughout the hearing care pathway.
- Monitoring and tracking
each child's progress to ensure timely access to diagnosis,
intervention and ongoing care.
- Coordinating
appointments and facilitating access
- to
diagnostic, rehabilitation and early intervention
services.
- Liaising
with audiologists, ENT specialists, Hearing Australia, early
intervention services and disability support providers to
ensure coordinated care.
- Supporting families in
preparing for appointments and understanding recommendations
from health professionals.
- Assisting families to
access appropriate supports, including early intervention
services, NDIS and community resources where
relevant.
- Participating in
multidisciplinary discussions and contribute to family-centred
care planning.
- Maintaining accurate
clinical records, data management and reporting in line with
National Universal Newborn Hearing Screening Program
requirements.
- Contributing to quality
improvement activities and evidence-based service
delivery.

professional with advanced practice skills, relevant paediatric
experience, and an understanding of childhood hearing loss and the
diagnostic, medical, rehabilitation and early intervention pathways
available to we encourage you to apply.
Experience working within
paediatric or neonatal audiology services is desirable but not
required.
An Allied Health
qualification is required for this role together with relevant
professional accreditation/eligibility for membership with the
relevant professional association as per the Job Description Form
(linked above).

our Team
The WA Newborn Hearing
Screening Program oversees universal newborn hearing screening
across Western Australia, including both public and private
hospitals.
We are a collaborative
statewide service committed to the early identification of hearing
loss and improved outcomes for children through timely diagnosis,
intervention and coordinated care. We work closely with health
services and community providers to ensure infants receive
appropriate and timely support throughout their care
journey.
Our team coordinates
statewide screening outcomes and referrals, provides training,
education and support to screening staff, facilitates access to
diagnostic audiology and follow-up services, and identifies and
monitors infants with risk factors for hearing loss. We also
support families from referral through diagnosis and intervention,
ensuring a consistent, family-centred approach to care across the
state.
