This is a recruitment process for Permanent Full Time / Part Time, Fixed Term Full Time / Part Time and Casual appointments with the possibility of extensions and/or permanency.

We are seeking a motivated Senior Occupational Therapist to join our multidisciplinary team.

Day in the life of a Home Hospital therapist

As part of the Home Hospital Allied Health team, you will play a key role in delivering acute and subacute care to patients in the comfort of their own homes, supporting clinical pathways that mirror inpatient care. No two days are the same. You’ll receive referrals from medical, nursing and allied health ward staff, with allocations made daily to support responsive service delivery. Working in a trans professional model, you’ll complete comprehensive functional and psychosocial assessments and provide timely reviews either in the Home Hospital clinic or through home visits.

In this dynamic role, you will lead complex discharge planning, connect patients with appropriate community supports, and provide essential carer education, coaching and safety planning. You will also contribute to multidisciplinary case coordination and communication, ensuring patients receive safe, person‑centred care that enables recovery at home.

South Metropolitan Health Service (SMHS) Home Hospital is a fast‑growing service delivering inpatient‑level care in the community. We currently operate 40 virtual beds and are expanding to 80 beds across the Fiona Stanley Hospital, Rockingham General Hospital and Peel Health Campus catchments.

Home Hospital enables patients to receive hospital‑equivalent care at home, reducing inpatient admissions while maintaining safe, timely and person‑centred care. Allied Health is essential in functional assessment, rehabilitation, complex discharge planning, therapeutic intervention and holistic patient support.

  • Confidence working both autonomously and collaboratively within a multidisciplinary team
  • Excellent communication skills and the ability to thrive in dynamic, mobile and virtual care environments
  • A commitment to safe, person-centred and evidence-based practice
  • Motivation, flexibility and adaptability within a fast-evolving service model
  • Passion for home-based, patient-centred care and holistic support

Experience in acute care, community practice or hospital substitution services is desirable, but not essential.
