This is a Fixed-Term, Full-Time
position lasting six months, with the possibility of extension
and/or permanency.

Whilst this selection process will initially be used to fill the

from this process. Both of these options remain valid for a period
of twelve (12) months from when the authorised delegate endorses
the recruitment decision.
Position Profile: We are currently seeking a
dynamic and enthusiastic individual to appoint to a Clinical Nurse
Specialist position that has become available with the Midland
Community Mental Health service. In this position, the successful
candidate will be responsible for contributing and overseeing
delivery of quality patient care within their area of speciality.
Should you be successful, you will work collaboratively to improve
and promote desired outcomes for patients through the development
of standards of practice, implementation of changes to clinical
practice, initiating research and quality improvement activities,
and the provision of a consultation service within your area of
expertise/assigned area to all customers within the Hospital and
Health Service. Key duties of this position include, but are not
limited to;
- Works collaboratively within the multidisciplinary team to
provide coordinated and integrated nursing care for mental
health consumers within a Care Coordinator Framework with
non-government agencies, community services, mental health
services and community accommodation services;
- Provides coordinated care of an allocated case load,
undertakes as required, comprehensive primary mental health
triage and assessment of consumers in the clinic and community
settings providing a range of community mental health
interventions using a variety of skills and therapies;
- Provides expert clinical consultancy and liaison service
for a broad range of stakeholders internal to health to assist
with physical health screening and preventative
activities.

Area Profile: The East Metropolitan Health Service (EMHS)
comprises an extensive hospital and health service network that
aims to maintain and improve the health and wellbeing of more than
670,000 Western Australians in its catchment area.
Our hospital network works together to provide a combination of
tertiary, secondary and specialist health care services including
emergency and critical care, state trauma, elective and emergency
surgery, general medical, mental health, inpatient and outpatient
services, aged care, palliative care, rehabilitation and women’s,
children’s and neonates services.
The EMHS consists of Royal Perth Hospital; Armadale Health Service;
Bentley Health Service; Kalamunda Hospital; St John of God Midland
Public Hospital and a range of community and population health
services, including the
Midland Community Mental Health Service.
The Midland Community Mental Health service provides specialist
clinical mental health services to individuals aged 18-65.
The specialists treat those:
- Who are not responding well to treatment for mental illness
in a primary care setting (e.g. General Practitioners)
- With severe and complex mental health issues/illness
- Have a significant level of disturbance and psychosocial
disability due to their illness that has a significant impact
on their functioning
- Significant functional difficulties that require
coordination of care and support across multiple agencies,
family, friends, support people, carers and providers
- Would benefit from a multi-disciplinary team due to the
intensive or complex nature of care required.
