The
Older Adult Health Hubs are a new model of care,
providing safe and quality care for older adults through
integrated, community-based intervention. This specialist-led older
adult hub delivers and integrates with services that focus on
proactive and responsive care, risk prediction and early
intervention.
The
SMHS Older Adult Health Hub:
- Optimises older adult
independence through maintaining and improving function,
growing health literacy, supporting choice autonomy and
scaffolding and supporting carers
- Provides
coordinated, comprehensive multidisciplinary care
- Supports
liaison between primary care and SMHS services
SMHS delivers
quality, safe and effective hospital and health services within a
catchment area stretching more than 3300 square kilometres across
the southern half of Perth.

Position
Description: The Care Navigator for the Older Adult Health
(OAH) Hub provides an integrated and seamless care pathway for
older adults by leveraging existing and emerging services to
facilitate coordinated care and reduce unplanned hospital
presentations. The role will actively network and collaborate with
key stakeholders across the continuum of care, including acute,
sub-acute, and community settings, to support vulnerable older
adult patients. This includes delivering education, fostering
liaison, and building strong relationships across services to
enhance outcomes for this client group. The Care Navigator will
actively engage in development and implementation of an evaluation
process, including recording, collecting and reporting key
performance information as well as conduct research activities in
line with the hub development and future planning. The Care
Navigator provides an advanced clinical service in triage and
screening referrals to the OAH Hub and supports linkage to existing
outpatient and community-based services within the South
Metropolitan Health Service (SMHS) and wider metropolitan health
services when indicated.

must have a valid working visa (for fixed term appointments) or be
an Australian citizen, permanent resident, or have a visa to live
and work indefinitely in Australia (for permanent
appointments).

months from when the authorised delegate endorses the recruitment
