Metropolitan Health Service (SMHS) is seeking an exceptional leader
for the pivotal and career‑defining role of Service
Director, Community Services.
This is a
Permanent Full‑Time appointment and a rare
opportunity to help shape the future of health care delivery in
Western Australia at a time of significant reform. At SMHS,
Community Services are a critical part of this
transformation—enabling people to receive high‑quality inpatient
and outpatient care in their own homes and community, while
strengthening integration across the health system.

leader to drive this agenda forward.
About
the role
As
Service Director, Community Services, you will
provide strategic leadership and operational oversight across SMHS
Home Hospital and a diverse portfolio of metropolitan and statewide
community‑based services. You will be responsible for shaping
service models that improve patient experience and outcomes,
strengthen system integration, and deliver safe, efficient and
sustainable care outside of traditional hospital settings. You will
lead multidisciplinary teams delivering complex care across home
and community environments, ensuring strong clinical and corporate
governance, and a culture that empowers staff to deliver
exceptional care consistent with SMHS safety, quality and
performance frameworks.

Statewide Care Navigation Service (VAD SWCNS)
SMHS is
strengthening integrated leadership across its hospital and health
service network, with the Chief Executive intending to establish a
Chief Operating Officer role. The Service
Director, Community Services will report to this role and play a
key part in advancing shared performance, service integration and
whole‑of‑system objectives across SMHS.
SMHS is also
on an exciting journey to become a world‑class leader in
continuous health care improvement. Through our
partnership with Virginia Mason Institute, we are
building a unified, future‑focused health system that empowers
every staff member to improve care, every day. The Service
Director, Community Services will champion the SMHS

Are
you eligible? To be eligible for appointment, applicants
must have a valid working visa (for fixed term appointments) or be
an Australian citizen, permanent resident, or have a visa to live
and work indefinitely in Australia (for permanent
appointments).

WA Health

Whilst this
selection process will initially be used to fill the above vacancy,
it may also be used to fill other "similar" vacancies throughout
the health service. In addition to this, should the successful
applicant decline or vacate the advertised vacancy, then the next
most suitable applicant may also be selected from this process.
Both of these options remain valid for a period of twelve (12)
months from when the authorised delegate endorses the recruitment
decision.
