Help Shape the Future of Care Beyond Hospital Walls

South Metropolitan Health Service (SMHS) is seeking an exceptional leader for the pivotal and career‑defining role of Service Director, Community Services.

This is a Permanent Full‑Time appointment and a rare opportunity to help shape the future of health care delivery in Western Australia at a time of significant reform. You will lead multidisciplinary teams delivering complex care across home and community environments, ensuring strong clinical and corporate governance, and a culture that empowers staff to deliver exceptional care consistent with SMHS safety, quality and performance frameworks.

Our teams deliver a broad range of services beyond bricks‑and‑mortar hospitals, including:

Across the South Metropolitan area

  • SMHS Home Hospital (HH)
  • Older Adult Health Hub (OAHH)

Across the Perth Metropolitan area

  • Rehabilitation in the Home (RITH)
  • Community Physiotherapy Services (CPS)
  • Complex Needs Coordination Team (CoNeCT)
  • Complex Needs Coordination Team – Mental Health Expansion (CoNeCT MHE)

Statewide

SMHS is strengthening integrated leadership across its hospital and health service network, with the Chief Executive intending to establish a Chief Operating Officer role. The Service Director, Community Services will report to this role and play a key part in advancing shared performance, service integration and whole‑of‑system objectives across SMHS.

SMHS is also on an exciting journey to become a world‑class leader in continuous health care improvement. Through our partnership with Virginia Mason Institute, we are building a unified, future‑focused health system that empowers every staff member to improve care, every day. The Service Director, Community Services will champion the SMHS Improvement Method, fostering a shared way of learning and leading that reflects our commitment to being Collectively One, Locally Led.

If you are a strategic leader with a passion for community‑based care and system transformation, we invite you to consider this exceptional opportunity.
