We are seeking motivated Senior
Physiotherapists to join our multidisciplinary
team.
Day in the life of a Rehabilitation in
the Home (RITH) therapist
As
part of the RITH team, you will play a key role in delivering
subacute rehabilitation to patients in the comfort of their own
homes. No two days are the same. You’ll receive referrals
from allied health ward staff, with allocations made daily to your
caseload to ensure patients are seen within one day of hospital
discharge. Working in an interdisciplinary model, you’ll complete
comprehensive multidisciplinary assessments before working with the
patient to establish their rehabilitation SMART goals.
In
this dynamic role, you will experience a diverse range of
rehabilitation categories, including elective joint replacements,
amputees, complex neurology, general reconditioning post extended
hospital stay and many more. You will work with our
Multidisciplinary team on complex discharge planning, connecting
patients with appropriate community supports, and provide essential
carer education, coaching and safety planning. You will also
contribute to multidisciplinary case coordination and
communication, ensuring patients receive safe, person-centred care
that enables rehabilitation at home.
You’ll have access to a work vehicle, laptop,
mobile phone and safety systems to ensure your home visits run
smoothly and safely.
South Metropolitan Health Service (SMHS) RITH is
a well-established early discharge service. We currently have
8 bases across the metropolitan region. You will be supported
by a Coordinator and P3 neuro physiotherapist, in addition to your
peers, for the more challenging clinical and non-clinical aspects
of your role. Regular professional development is part of a
standard week, with opportunities for quality improvement,
innovation and career development on offer. The
within-team support from Occupational Therapists, Dietitian, Social
Worker, Speech Therapist, Medical Registrar and Allied Health
Assistants will keep you inspired and keen to go to work each
day.

Strong clinical reasoning, assessment and
problem-solving skills, with experience in both acute settings and
rehabilitation.

Excellent communication skills and the ability
to thrive in dynamic, mobile and virtual care

where you have had conflict with a team member and how you

- Identify a contemporary
clinical issue in health services or physiotherapy practice
and outline what steps you would take, or have taken, to
develop a quality improvement initiative to address
this.
- Please provide the names
and contact details of two (2) referees, preferably one
being your current line manager
