This is a Recruitment for Permanent Full Time & Part Time, Fixed Term Full Time & Part Time appointments, with the possibility of extension(s) and/or permanency.

We are seeking an experienced and committed Senior Social Worker to join our NDIS Linkage Team, providing advanced clinical expertise, leadership, and high‑quality support to participants navigating the National Disability Insurance Scheme (NDIS) and related community services.  In this key leadership position, you will work with individuals, families, and carers to deliver comprehensive social work assessments, complex case coordination, and specialist guidance aimed at improving access, independence, and wellbeing.

The successful applicant shall practice in accordance with the Australian Association of Social Workers Practice Standards 2013 and Code of Ethics 2010 and/or the Society of Professional Social Workers Code of Ethics and SMHS policies and guidelines. As part of the NDIS Linkage multi-disciplinary team, provides advanced Social Work clinical support & knowledge within the team, was well as across the organisation for Fiona Stanley Fremantle Hospital group, including the State Rehabilitation Service. Acts as a Social Work NDIS clinical consultant/resource to Social Workers and other health professionals. Plans, implements, and evaluates patient safety and quality of care.

Fiona Stanley Hospital (FSH) is a 783-bed tertiary hospital offering a range of health services including the State Rehabilitation Service, a comprehensive cancer centre, purpose-built mental health unit, maternity, paediatric and neonatal services, and the State Adult Burns Service. FSH also delivers specialised services such as heart and lung transplantations and hyperbaric medicine.

SMHS delivers quality, safe and effective hospital and health services within a catchment area stretching more than 3300 square kilometres across the southern half of Perth.
