Position Profile:
We are currently seeking to
appoint a Aged Care Liaison Officer OPI.
Works within the Older Person Complex Care
Team to support vulnerable older clients, their carers, families
and health professionals to provide appropriate assessment and
follow up. Assist with the
implementation of the age friendly principles and practices to
assure a high standard of service delivery for older patients
across the aged care continuum.
