Position
Profile:
Coordinate the strategic and
operational planning for referrals, admission and discharge to the
Hospice including patient transfer activities and associated
services for identified patients. Works collaboratively to improve
the quality of patient care and promote desired outcomes in the
patient population, which includes patients with complex needs.
Coordinate the implementation and provision of pathways that
facilitate smooth and effective transition to adult health services
for adolescent patients with a life limiting condition.

Sandcastles (Boodja Mia) Children’s
Hospice: Due to commence
operations mid-2026.This purpose-built, world-class paediatric
hospice, located on the stunning Western Australian coast, offers
statewide specialist paediatric palliative care services for
children and young people (0–18 years) who are living with
life-limiting or life-threatening conditions, and their families.

collaboratively I within a multidisciplinary team to negotiate
a positive outcome for a patient and their family/care givers.

- Working in hospice and
palliative care requires sensitivity and high-level critical
thinking. Please provide a specific example that demonstrates
your advanced communication and problem-solving skills when
managing a complex clinical assessment, navigating a sensitive
family concern, or addressing a challenging operational issue
(such as bed availability or workload). (in response to
Essential Criteria 3 & 4)
- Can you share an example
of how you have applied quality improvement principles to
enhance patient care or service delivery?
- Provide
a current resume which summarises your experience and includes
both phone and email contact details of two professional
referees, with at least one being a recent supervisor/line
manager.
