Position Profile: We are
looking for professional, motivated and experienced Clinical
Nurses to join our team on Ward 2B – Long Stay Surgical,
complex ENT, Gastroenterology and Gynaecology ward at the Perth
Children’s Hospital with the Child and Adolescent Health
Service (CAHS).
This role
will be responsible for ensuring the delivery of comprehensive
evidence-based nursing care to our young patients. Key
responsibilities will include:
• Providing
comprehensive evidence-based nursing care and individual case
management to a specific group of patients including
assessment, intervention and evaluation.
• Undertake
clinical shifts at the direction of senior staff and the
Nursing Director including participation on the
on-call/after-hours/weekend rosters if required.
•
Responsible and accountable for patient safety and quality of
care through planning, coordinating, performing, facilitating
and evaluating the delivery of patient care relating to a
particular group of patients, clients or staff in the practice
setting.
• Monitor,
review and report upon the standard of nursing and midwifery
practice to ensure that colleagues are working within the scope
of nursing and midwifery practice, following appropriate
clinical pathways, policies, procedures and adopting a risk
management approach in patient care delivery.
•
Participate in ward rounds/case conferences as
appropriate.
• Educate
patients/carers in post discharge management and organise
discharge summaries/referrals to other services, as
appropriate.
