What
is our Clinical Nurse role?
The
successful applicant will work as part of a multidisciplinary team
to provide clinical and professional expertise to ensure
comprehensive, evidence-based nursing care and specialised youth
mental health interventions are delivered to young people in a
community setting. This includes providing intensive community
outreach, incorporating assessment, risk assessment, risk
management, care planning, case management, crisis interventions,
and time-limited care coordination to support young people with
complex mental health needs, along with their families and
carers.
