About the
Role
The
Child and Adolescent Health Service (CAHS) is seeking a
Service Coordinator – Family Support and Outreach
to join our Paediatric Palliative Care and Hospice Service
(PPC&HS) and lead and support the delivery of
evidence-based psychosocial care for children and young people with
life-limiting conditions, and their families, across hospital,
community and outreach settings.
Reporting to the Program Director - PPC&HS,
you will manage a small multidisciplinary team and work closely
with clinical and community partners to ensure families receive
timely, coordinated and culturally safe support throughout their
journey - from diagnosis through treatment, end-of-life care, and
complex grief and bereavement.
With
a focus on identifying their psychosocial needs, you will deliver
tailored, evidence-based interventions to support the whole
family including siblings and extended family members. This
includes offering practical assistance such as navigating NDIS and
community services.
You
will also provide statewide specialist consultation, liaison and
supervision to health professionals and partner agencies supporting
children and families in palliative care settings, including
regional and rural areas, strengthening the capacity of the broader
system to deliver family-centred care.
Key
responsibilities include
- Leading your team in
delivery of a broad range of psychosocial interventions,
including comprehensive assessment, allocation, and case
management.
- Engaging with families
across diverse community contexts, ensuring equitable access
through proactive outreach to rural and remote
areas.
- Collaborating with schools,
community services, NGOs and other agencies to support children
and families close to home, including rural and regional
outreach, and advocate for family needs throughout the care
continuum.
- Contributing to service
development, policy, governance, quality improvement and
performance reporting.
- Participating in workforce
planning, recruitment, supervision and professional
development.
- Working collaboratively with
clinical leadership to align service delivery with strategic
objectives, budgets and performance targets.

About the Sandcastles
(Boodja Mia) Children’s Hospice:
Due
to commence operations mid 2026, this purpose-built, world-class
paediatric hospice, located on the stunning Western Australian
coast, offers statewide specialist paediatric palliative care
services for children and young people (0–18 years) who are living
with life-limiting or life-threatening conditions, and their
families.
This
state-of-the-art, 7-bed facility will be the first of its kind in
Western Australia, providing specialised palliative care for
children living with life limiting conditions aged from birth to 18
years within a peaceful, home-like setting that prioritises
comfort, dignity and connection.

team cohesion and performance? Provide an example from your
experience.
