The Role

As Transition Coordinator, you will lead the student transition program. This role involves coordinating key activities such as parent information sessions, Masterclasses and Orientation programs to support students aged 15 to 18 as they transition into the college. You will provide individualised case coordination and brokerage (referral) services, offering guidance to students, families and staff and connecting students with the most relevant external providers based on their unique needs. Active participation in multidisciplinary team meetings and collaboration with other agencies is essential, along with providing consultancy advice and support as required.

In addition to supporting individual transitions, you will research, develop, implement, and monitor high-quality plans that enable young people to access suitable pathways to recreational, training and employment opportunities. You will assist in developing systems and intervention strategies that deliver responsive and effective support services, while maintaining strong networks with local youth, education, employment, and counselling services. The role also includes maintaining accurate records, analysing data, and preparing reports to support the development and monitoring of individual plans.   5. Demonstrated well developed oral, written and interpersonal communication skills with the ability to establish and maintain effective working relationships, negotiate and resolve conflict.
