Position Profile: The Child and Adolescent Health Service are seeking experienced and dynamic individuals to join our team within the Immunisation Service at Perth Children's Hospital.
Successful candidates will, in collaboration with the Clinical Nurse Manager, provide clinical and professional expertise to ensure comprehensive evidence-based care and advice is delivered to patients and families in relation to immunisation. This includes counselling and education of patients and families with complex medical conditions in relation to immunisation.
Reporting to the Clinical Nurse Manager, your responsibilities will include:
- Acting as an advocate for patients and families and collaborating with them and other members of the health care team to achieve health goals.
- Demonstrating competence in the planning and delivery of complex care to patients and their families.
- Liaising with members of the multi-disciplinary team to resolve complex patient problems and evaluate care.
- Participating effectively as a member of the multi-disciplinary team and fostering continual development of the team to provide expert services.
- Supervising and acting as a resource for staff in the assessment of patients and development of Immunisation catch up plans.
