We are seeking a compassionate and dedicated Welfare Officer to join our community services team. In this rewarding role, you will provide support, guidance, and advocacy for individuals and families, helping to improve wellbeing and access to essential services. The successful applicant shall provide a Welfare Officer service to Complex Needs Coordination Team (CoNeCT) to meet the needs of individuals with complex medical and social needs, their families and carers and other consumers of the service. Assesses and prioritises patient welfare needs and provides direct assistance to access benefits and other requirements. Liaises with government and other service providers to provide necessary assistance. Works in conjunction with a multidisciplinary team in providing a comprehensive social welfare service for patients.
