Position Profile: An opportunity exists for a Senior Social Worker to join our team in Halls Creek. In this advanced practice role, you will plan, coordinate, deliver and evaluate timely, culturally appropriate social work services across Halls Creek and surrounding communities. Working within a primary health care framework, you will support the promotion, maintenance and improvement of health and wellbeing for individuals and families, providing clinical social work services across both community and acute care settings. The position plays a key role in care coordination and discharge planning for clients with complex psychosocial needs, while also delivering secondary consultation, professional support and expert advice to multidisciplinary teams and regional healthcare staff. Suitability Criteria: Suitability recognises that a range of factors contribute to merit, including skills, knowledge, abilities, behaviours, ethical alignment and relevant operational requirements. The suitability requirements below are derived from the Job Description Form and define what will be assessed to determine your suitability for the role:

1. Tertiary qualification in Social Work and eligible for full membership of the Australian Association of Social Workers.
2. Demonstrated experience and advanced level of skills and knowledge in the planning, implementation and evaluation of Social Work services in primary health care, outpatient and acute settings, across the lifespan.
3. Demonstrated knowledge and understanding of primary health care principles and practice.
4. Demonstrated clinical leadership and governance in Social Work services and practice.
5. Demonstrated leadership working in a cross-cultural setting, considering cultural and social determinants, particularly as they relate to Aboriginal health.
6. Demonstrated personal, team and program/department managements skills with communication skills.
