Position Profile: WACHS South-West is looking for experienced clinical pharmacists to join the Bunbury team. These roles promote safe, rational and cost-effective medication therapy by providing clinical pharmacy services for hospital patients throughout the South West region. This includes, but is not limited to, medication reconciliation for admitted hospital patients, monitoring the patient’s total medication profile, dispensing medications to patients, providing drug information to nursing and medical staff and making independent decisions on therapeutic strategies to optimize patient care.

Your practice as a Pharmacist will align to the Pharmacy Board of Australia Registration Standards, the Pharmacy Board of Australia Codes and Guidelines including the Pharmacy Code of Conduct for Registered Health Practitioners and WACHS policies and guidelines. 

This is a Permanent Full Time, Permanent Part Time, Fixed Term Full Time or Fixed Term Part Time appointment, with the possibility of extension(s) and/or permanency.

Suitability Requirement 4 : Demonstrated commitment to continuous learning, reflective practice and quality improvement to maintain contemporary clinical expertise and contribute to service development and improved patient care outcomes.

Suitability Requirement 5 : Demonstrated ability to work effectively and independently within a healthcare service, adapting to diverse clinical settings, managing competing priorities and supporting pharmacy service delivery.

Suitability Requirement 6 : Provide evidence of your tertiary qualification in Pharmacy and current general registration with APHRA.

Suitability Requirement 7 : Provide evidence of your current C or C-A class drivers licence

• A detailed curriculum vitae (CV) summarising your relevant experience and ability to meet the position requirements
• The names and contact details of two (2) professional referees, preferably including your current or most recent manager
