The Armadale Youth & Community Services Hub is seeking a dedicated and supportive Social Worker. As a full service school, the Youth & Community Services Hub, located on the Armadale Senior High School (SHS) campus, provides assistance to young people with complex needs; providing targeted assistance within areas such as mental health support, social services, sourcing temporary accommodation, parenting support, life skills training and specialist health services. As a Social Worker within this unique context, successful applicants will actively participate and contribute to the development of a positive culture aimed towards providing appropriate services for young people and their family; which could include completing assessments, planning and implementing intervention programs as well as providing opportunities for success.
Staff within the Youth & Community Services Hub have an authentic desire to support young people who may be at risk. We are seeking applicants who ideally have the ability to work well within a multidisciplinary team, delivering a high standard of evidence-based care to all clients. As the Youth & Community Services Hub attracts referrals from five local secondary schools, suitable applicants should have excellent interpersonal skills, including the ability to establish and maintain effective working relationships with staff, young people and the wider community.
About the Full Service School
The Youth & Community Services Hub works to maximise opportunities for students beyond the standard classroom. These young people may have been affected by a number of external factors (such as family circumstances, poverty, health, cultural differences or disengagement) and require additional support. The Role
In conjunction with the Program Coordinator, Social Workers within the Youth & Community Services Hub undertake assessment of referred students, prepare plans and undertake social work intervention with young people and their families, whilst interviewing parents/guardians, to obtain and evaluate case information from all relevant sources, and make external visits as appropriate. They should work flexibly within changing environments using a well-developed understanding of trauma-informed models of care to provide targeted support, acting as a link between the young person and other appropriate agencies within the community. With an effective case management mindset, Social Workers develop appropriate support plans for young people that have complex needs.
A key aspect of this role is providing outreach support to vulnerable young people across the Armadale, Kelmscott and Byford regions. Social Workers aim to support their young people in a range of environments by developing innovative solutions to their problems and by providing access to appropriate social services. As such, suitable applicants will ideally have a strong ability to build and maintain rapport with the people they work with; building trust through the use of open communication; and adapting methods of engagement to meet their individual needs.
The successful applicant will act as a resource person within the school community and participate in school and community projects whilst participating in multi-disciplinary team meetings and case conferences within school and other agencies as appropriate. You will consult and advise other staff on matters related to specific cases whilst organising and maintaining a records system, collate statistical data and provide annual and case reports, as required. The successful applicant will assist with appropriate submissions on school social work-related issues.
